Centralize locking with ust_lock, fork handling

A single lock is held across forks (ust_lock), which synchronizes _all_
operations performed on UST structures.

- cleanup/reinit UST after fork in child.
- ensure to unlock URCU-bp mutex before calling synchronize_rcu again.
